Cleanup regarding the dependency upgrade to dune 2.4.

* Use of initializer lists for FieldVectors
* Read vectors from input file
* General formatting and white space
* Remove unused typedefs, variables, files

The enthalpy of the injected CO2 is now evaluated using the
actual pressure and temperature at the injection well.

[parkervangenuchten] Use the new ParkerVanGenuchten material laws

The 3p ParkerVanGenuchten material laws have been revised. The applications
now use these classes.

Added dune_enable_all_packages() that was missing
and prevented compilation.

* introduce epsilons for float comparison for robustness
* the grid file is no longer needed
* read vectors from input file
* simplified many methods
* combined the threep and threepthree3 in one file for better maintainability
  introduce a preprocessor switch

It remains to include epsilons for the water table and compare
results with the reference vtu currently failing the fuzzy compare.
Might need update. Looks good physically.

Some things could be written shorter with c++11 features.
Some formatting for better readability and maintenance.

The check whether we are inside the lens has to use epsilons.
Otherwise floating point comparison is not robust. This patch
has the side effect that lenses can be specified much nicer,
as the points on the bounding box boundary are considered inside
the lens (which is what we would except in the examples.)

* FieldVectors take an initilizer list
* vectors can be read from input file
* improve doc
* whitespace cleanup
* indent and formatting cleanup
* remove unused typedefs

Instead of copying it's content on every change.
Format nicely.

Disable committing trailing spaces and tabs.
