Fixed wrong reference in handbook: The problem description (treatment of Boundary Condition) in the decoupled tutorial references common parts of the coupled tutorial. To indicate where the description in the coupled part becomes model-specific again, the phrase "box models" was added.

removed TypeTage from thermalconductivitysomerton.hh, because material laws should remain property-free.
Thus, the arguments of the effectiveThermalConductivity method have been changed to Scalar quantities only.
Deprecated boundaryMatrixHeatFlux method, which is not used anymore.

There I introduces a bug by naming a loop variable that was already
declared. Kudos to fetzer for doing all the work and finding this bug.
The Stokes models should convert better again.

The "standard implicit" way of determining phase compositions is by means of the MiscibleMultiPhaseComposition constraint solver. 
The constraints are written into a matrix and the matrix is solved. 
However, if it cannot be solved an exception is thrown. 
This commit catches that exception, and throws NumericalProblem so the simulation can try and continue.
